Prioritizing Client Care
Leadership, Delegation, and Emergency Response Planning
患者护理排序:领导、授权与应急计划
A professional nurse is both a leader and a manager. He or She needs to provide integrated, cost-effective care to clients by coordinating, supervising, and collaborating or consulting with members of the multidisciplinary health care team. Prioritizing client care is an important step in achieving this goal. Main points of review may therefore focus on the skills required to prioritize client care activities and understanding and application of the concepts of management, case management, quality improvement, delegation, emergency response plan, and triage.
As usual, tips on review of prioritizing client care will be discussed in three parts:
1. Nursing delivery systems and prioritization of client care;
2. Leadership and management; and
3. Emergency response plan and triage.
Part 1
Nursing Delivery System and Prioritization of Care
护理服务体系及患者护理排序
Never before has there been such a need for nurses to work competently and collaboratively with other healthcare professionals to secure cost-effective, high-quality healthcare for all. To do all this successfully, nurses must be familiar with the healthcare organization in which they are employed, as well as the healthcare system at large.
A. Nursing Care Delivery Systems护理服务体系
A nursing care delivery system defines how work is organized, how nursing staff are deployed, who does what in providing nursing care, and who has the accountability for nursing care and clinical outcomes.
1. Functional nursing – 功能性护理, in this system, the charge nurse (护士长,主管护士) delegates specific tasks to each member of the team; team members focus on the delegated task rather than the total client. It is a task-oriented system. The disadvantage of this system is that it results in fragmentation of care (护理片断化).
2. Team nursing – 小组护理in this system, the registered nurse functions as a team leader and determines the work assignment; care is given by a team composed of registered nurses (RNs), Licensed Practical (or Vocational) Nurses (LPNs), and certified nursing assistants (CNAs); most direct bedside care is given by the CNAs while the RNs spend more of their time at the nursing station.
3. Modular nursing -- 模块护理In this system, the unit is divided into modules so that nurses care for a group of clients who are geographically close by. It is similar to team nursing, but it takes into account the structure of the unit.
