    Nurse scientist or nursing scientist: Future considerations for the field p523–524

    The history of the development of nursing science is well into its seventh decade. To say that “We have come a long way”, is an understatement. However, the pipeline for nurse scientists (i.e. PhD prepared nurses) to continue to sustain and advance the discipline has stabilized well below the projected need across the U.S. (AACN, 2015). Despite modest gains in increasing numbers of PhD students enrolled and numbers of PhD programs, in 2014 there were 18, 352 students enrolled in 264 DNP programs, while only 3,065 students were enrolled in 134 PhD programs.

    I feel as if we are finally acting on evidence that has been known for some time; there is significant value to enhancing the contributions of registered nurses practicing in primary care. The outcome of increased efforts to engage RNs in primary care practice could be gains in the health status of patients, reduced dissatisfaction of providers (physicians, nurse practitioners, and physician assistants), and an improved value proposition for primary care.

    I can't express how thrilled I was with the opportunity to read Lindell Joseph's and Richard Bogue's article (July, 2016), “A theory-based approach to nursing shared governance”. It is perhaps one of the more comprehensive and significant study activities related to shared governance that I've had the opportunity to review in my 45 years in nursing, 35 of them involved in shared governance at any number of levels from conceptualization, implementation, and evaluation. I'm especially pleased to see this level of enthusiasm and thoroughness in examining theory and process related to shared governance after 30+ years of experience with its explication and application.

    I was delighted to read about the theoretical foundation of nursing shared governance in “A theory-based approach to nursing shared governance” (Joseph & Bogue, 2016). Academics rarely publish in this sphere, and I am hopeful that others will soon follow. However, I wanted to offer more accurate representations of my work in shared governance over the last 30 years, particularly in regard to the Index of Professional Nursing Governance (IPNG) and the Index of Professional Governance, which have become the standard instruments for measuring shared governance.

    In recent months, the mosquito-born Zika virus and potential complications associated with transmission of the virus has been at the forefront of much public discussion and angst. The potential congenital malformations associated with contracting Zika during pregnancy as well as the risk for serious autoimmune and neurologic sequela in affected persons have been highlighted in most news outlets (Marcus, 2016,Searcey, 2016).

    The Centers for Medicare and Medicaid Services (CMS) has differential payment for inpatient and observation status patient services. Observational stays are not credited toward the admission criteria for a skilled nursing facility (SNF). This rule poses financial concerns and burdens for Medicare patients. The Academy supports efforts to amend and replace the current payment rules with a more equitable payment policy for observation status and to limit its financial liability on patients.

This session was recorded and is posted on YouTube https://www.youtube.com/watch?v=BNPz852gWLs as well as on the Council for the Advancement of Nursing (CANS) website. Among the plenary speakers and topics throughout the two and one-half day meeting were Dr.
